Accessing Support at Home

Crisis Response is a service operated by West Lothians Health and Social Care Partnership to assist people who have fallen within their own home or to respond to telecare alerts.

The Support at Home Service incorporates the Crisis Response, Home Safety Service, Re Ablement, Falls Assessment and Prevention .

Use the Self-Assessment to understand your capabilities, access tailored advice, and complete an onward referral.

Use this form if you have already had an assessment or are waiting for support and you need to update us on a significant change to your circumstances, including an increase in risk or need.

Use this form if you currently receive a service and are looking for it to be reviewed and make significant changes to your support, e.g. decrease or increase your support or request a change of care provider.
