Community Psychiatric Nursing Elderly (CPNE) Older Adult Team
The CPN Team work to reduce the need for patients to come into hospital by providing a comprehensive assessment and care management plan for individuals and their families.
The aim would be to provide information and treatment, and to promote wellbeing and recovery in the community setting.
