Referral from health to WLC community based settings

This form is for health staff to make referrals to the Integrated Access Point, which is a multidisciplinary access team covering Social Work, Community Occupational Therapy, District Nursing, and Community Rehabilitation, or to the Intermediate Care Service, which covers Support at Home Services, and the Integrated Discharge Hub at St John's Hospital.
