Macmillan Improving the Cancer Journey
Cancer doesn't just affect your physical wellbeing, it can affect every aspect of your life, from your emotions to your finances.
Often, people don't know where to turn for support. That's why West Lothian Council, NHS Lothian and Macmillan Cancer Support have launched the Improving Cancer Journey (ICJ) Service.
Our Service is here to help people affected by cancer get the support they need, whether it's financial, emotional, medical or practical.
Who we can help?
Improving the Cancer Journey is a service available to anyone affected by cancer in the West Lothian Council area.
When we can help?
ICJ is available at any stage of someone's cancer journey
How can we help?
Once an individual gets in touch we will arrange for them to meet with a dedicated Macmillan ICJ Link Worker who will help them complete a Holistic Needs Assessment (HNA).
This is a tool which can help people to identify and discuss their needs and concerns. Using the HNA, the Macmillan ICJ Link Worker and individual will work together to decide the best kind of support they and their family want and create a personalised care plan.
Where can we meet?
The appointment can be carried out via telephone, video call or face to face in West Lothian Libraries and Partnership Centres
