Community Outreach Team (Social Work Team)

Social work involvement in the Community Outreach Team ensures that a holistic approach is taken to understanding the needs and difficulties of the individual (and their family) and places these needs within a social context.

This ensures that an individual is not seen solely in terms of their diagnosis and / or their need for medical treatment.

Social workers and the mental health worker act as lead workers and are involved in providing on-going support to clients.

They also work jointly with Community Psychiatric Nurses (CPNs) in the Team.  The interaction of the medical model and the social model can be seen to complement each other within the multi-disciplinary team thus providing clients with a holistic service.

  • Monday to Thursday 8.30am - 5pm
  • Friday 8.30am - 4pm
  • An answering machine operates outwith office hours
