Community Mental Health Team
The Community Mental Health Team West Lothian are a specialist mental health service made up of various professionals who assess, treat and support people experiencing severe complex mental health difficulties.
We work with people aged between 18-65 years who may be suffering from a mental disorder and require help with complex needs such as:
- Complex and persistent mental illness
- Complex problems of management and engagement
- Any mental disorder where there is a risk of self-harm or harm to others where the level of support requires a collaborative multi-disciplinary approach.
- Severe disorders of personality where these can be shown to benefit by continued treatment and care
Following assessment, if we feel we can help meet your needs we will allocate you a health professional who will manage your care. Your health professional will meet with you to create a holistic care plan that meets your goals and expectations. Often you will have multiple workers to help and support you. Over time they will assess, review, monitor your progress and link in with our wider team of professionals to offer you the best care available.
We aim to provide holistic care by reviewing, assessing and monitoring mental and physical health, your wellbeing, housing issues, financial circumstances, any substance misuse, personal history, reduce identified risks and help you meet your goals/needs.
Continuing care clinics:
This clinic (known by some as 'The depot clinic') is a safe environment for you to have your medication by injection. We have clinics running within our service area and satellite clinics in health centres. Staff will update you on options available for attending for your medication. We also offer home visits when required. People can have their medication and explore physical and mental health issues. People can also have appointments out with this clinic to develop a care plan and address social, emotional, financial and wellbeing needs with an allocated worker.
Clozapine clinic:
The Clozapine Clinic is a Nurse Led clinic. Based in OPD6, St John's Hospital it provides review of care for people on Clozapine medication. Blood testing and results will be given. We will also offer physical health checks.
Both Community Mental Health Team East and West clinics run jointly: Tuesday and Wednesday 9am until 3pm, OPD6, St Johns Hospital
