Re-Ablement

Re-ablement provides a short term assessment service, ordinarily for no longer than six weeks with reviews of your progress at regular intervals.  The approach that we take in re-ablement is that rather than carry out tasks for you, we will support and encourage you to do a range of daily living tasks needed for everyday life. The level of service we offer will vary depending on the progress you make during the assessment period. 

At the end of your assessment, and providing you meet the eligibility criteria for paid services (please see links to related documents), for example a care at home package we will discuss this with you as part of your reviews and look at the commissioning options that are available to you.
